Abdominal CT scanning is the gold standard diagnostic test if NOM is to be pursued. Appropriate timing of IV contrast is crucial to define the extent of splenic injury and identify vascular blushes. Controversy exists over the need for oral contrast to diagnose HVI and its potential for causing severe aspiration pneumonitis.38
